ASTM C478 — the manhole standard — contains no buoyancy provisions at all. It is a manufacturing spec, not a design spec. NPCA’s own Utility Structure Best Practices Manual names buoyant force as a load condition and supplies no method and no safety factor.
So every reviewing agency writes its own rule, and they disagree:

The fix, when it fails
If a structure doesn’t make the factor of safety, the report states the additional dead weight required and the base projection that achieves it. A rejection becomes a change order.
